How to apply
Subsidised (free for eligible low-income patients) transport for cancer patients travelling between home and hospital for treatment like chemotherapy. For Singaporeans/PRs who are cancer patients and receive MediFund, CHAS Blue/Orange, MFAC, or Public Assistance, or are known to a social worker. Ask your doctor, nurse, or medical social worker to submit the form.
