How to apply

Usually referred by a hospital discharge team or GP after a stroke, hip fracture, or other acute illness. Provides physiotherapy, occupational therapy, and speech therapy at a day rehabilitation centre. Subsidies available based on income. Call AIC at 1800-650-6060 to find a centre near you.
